I give permission for my child to participate in the Physical Education Class at the gym All Sports Courts (9623 25 Avenue NW, Edmonton, Alberta T6N 1H7, Canada) during school hours for the academic year of 2025/26. Students will be transported to and from the gym by AlBaqir Academy bus.
Accidents can occur with or without fault on either the part of the student, or the school, or the facility where the activity is taking place. My signature indicates that I am allowing my child to participate in this activity and to be transported to the event by AlBaqir Academy bus. I am accepting the risk of an accident occurring, and agree that this activity, as described above, is suitable for my child.
Please note that by signing the form, I allow AlBaqir Academy to transport my child by bus to and from the gym. Also, please note by this signing the form, the school and the staff are not liable for any accidents or injuries.
I hereby give permission in the event of injury for the Academy and authorities to administer or obtain first aid. In the event that we cannot reach you, we will transport your child to the nearest hospital for immediate medical care.
